| Bring a driver: At your first visit, the doctor will likely dilate your pupils. This involves using drops that make the pupil, the black part of the eye, big so that the doctor can examine your eye for such diseases as glaucoma, cataracts, diabetic changes, etc. Also, you may want to have a family member with you to help you remember the findings of the examination and to ask questions. |

| Bring a list of medications: It is helpful if you bring a list of your medications to your first visit. CLICK HERE to print our handy medication form which you can fill out ahead of time. |

Bring a photo ID: Please bring a driver’s license or other government-issued photo identification card. |

| Bring your insurance card and a referral, if required: Many insurance companies require a referral from the primary care physician listed on your insurance plan. A referral from another specialist or eye doctor does not qualify as a referral from a primary care physician. If you are not sure what is required, please stop by our office on a date prior to your first appointment. Bring your insurance card and we will help you determine if you need a referral. |

| Have sufficient time: Most initial visits require a significant amount of time. This is because it takes time to complete the paperwork, verify your insurance benefits, possibly get your eyes dilated, etc. |

| Special note: We try our best to limit the amount of time you spend waiting in our office. However, there are times when complex patient requirements or emergencies back up our schedule. We appreciate your patience during these times and you can be assured that you will get the same quality eye care as all our patients. |
