We look forward to providing you with excellent eye care. Below please find a list of items you will need to address for your first visit with us:

Insurance

Please bring your latest insurance card to your first visit. Many insurance companies require a referral from the primary care physician listed on your insurance plan. A referral from another specialist or a primary care doctor who is not listed as your primary care physician on your insurance may not qualify. If you are not sure what is required, please stop by our office on a date prior to your first appointment. Bring your insurance card and we will help you determine if you need a referral and form whom.

List of Medications

It is helpful if you bring a list of your medications to your first visit. CLICK HERE to print our handy medication form which you can fill out ahead of time.

Photo Identification

Please bring a driver’s license or other government-issued photo ID.

Bring a Driver

At your first visit, the doctor will likely dilate your pupils. This involves using drops that make the pupil, the black part of the eye, big so that the doctor can examine your eye for such diseases as glaucoma, cataracts, diabetic changes, etc. When dilated, you can expect your vision to be blurry for at least several hours. Also, you may want to have a family member with you to help you remember the findings of the examination and to ask questions.

Have Sufficient Time

Please know that most initial visits for a comprehensive eye examination require a significant amount of time in our office, sometimes a few hours. This is because it takes time to complete the paperwork, verify your insurance benefits, wait for your pupils to dilate, perform a detailed examination of your eyes, perform testing, explain your findings, and schedule any follow-up, laser, or surgery. We appreciate your patience during your exam and you can be assured that you will receive the best quality eye care we can give. For your convenience, we provide free WIFI.
